Give a service shop useful observations, not a guessed diagnosis. A clear timeline and description can help it decide what information or inspection is needed next.
Download printable guide & worksheet (PDF)Record what actually happened
Write down the date, mileage, warning indicators and the symptom in plain language. Note whether it began suddenly or gradually, whether it is constant or intermittent, and what changed shortly before it appeared.
Describe the conditions
Record whether the motorcycle was cold or warm, moving or stationary, and the relevant weather or road conditions. Describe the sound, feel, smell or visible sign without deliberately recreating an unsafe situation. Take photos or recordings only while safely stopped.
Give the shop a complete timeline
Provide service history, modifications, recent work and anything already tried. Keep fault-code text or numbers exactly as displayed, with context. Ask how the shop wants the motorcycle delivered and whether it should remain unused pending inspection.
What to record in your symptom & service request sheet
Use the second page of the PDF to keep these details together. Print another copy when you need a fresh record.
